The Ventilation Shaft extract location can be found west of the Streets of Tarkov. It is also across the Concordia Building.

The Ventilation Shaft extract location is beside the cinema or the theater. When you are facing the cinema, it should be on its right.
The Ventilation Shaft is the pentagon-looking stone on the right side of the cinema as seen in the screenshot below. Stand beside it to begin the extraction.
